National Bereavement Care Pathway

A tool kit that has been developed to enable healthcare professionals (GPs, Practice nurses, HCAs, Receptionists/Secretaries) to use and deliver the pathway effectively in the primary care setting. It explains about the National Bereavement Care Pathway and the role primary care has in supporting families in the community once they have been discharged from the hospital.

The tool kit contains:

· An information sheet explaining about the National Bereavement Care Pathway and the role primary care can play in supporting these families. The NBCP includes a set of five best practice care pathways for families who experience a miscarriage, termination for fetal anomaly, stillbirth, neonatal death or sudden unexpected death in infancy up to a year. · There is also a document containing template letters which have different versions depending upon the type of loss the family has experienced.  They have been amended to include both national and local Somerset based support charities tailored to the type of loss. These can be sent out to families by the GP  practice once they have received information from the hospital notifying them of pregnancy or baby loss.

Somerset Maternity Voices (MVP) have also developed a couple of Pregnancy & Baby Loss contact cards which practices may find useful for their families who have experienced the loss of a baby.
