Salaried GPs – North Sedgemoor Primary Care Network

North Sedgemoor Primary Care Network is looking for GPs to join our multidisciplinary team (One Team). These roles will support the delivery of the PCN DESs, particularly the Enhanced Care in Care Home DES and the Anticipatory Care DES. In addition the PCN is involved with the Complete Care Community Programme through NHS England with a focus on Learning Disabilities.
The key components of the role will be to:
Provide clinical support and leadership for the MDT
Develop the strategy for the delivery of the PCN DES in line with the overall Somerset strategy
Build relationships with all necessary stakeholders
We are ideally looking for clinicians with a background in complex care and a passion for MDT working.
12 sessions are available in any combination of days. Initially this will be a fixed term one year contract with potential scope to extend.
