The practice support team have developed a guide with protocols and templates for frailty to cover both the SPQS and contract requirements, all the documents are attached and the guides and leaflets are also available on the LMC website below.

We are unable to post the clinical templates online due to their format so please do file on your systems

We hope you find these useful and they will save practices some valuable time
