Wells Health Centre is looking for a highly motivated registered nurse to join our existing Practice nursing team.

Wells Health Centre  is a forward-thinking practice located in Wells . We pride ourselves for providing exceptional care with the right person in the right place at the right time. A Training Practice involved in the training of fully qualified doctors who wish to enter General Practice.

Job-type : Part-time, Permanent

Salary: Dependant on experience

Hours: 24 per week split over 3 days (Monday - Wednesday -Friday)

Job responsibilities:

The post holder will work as part of the primary healthcare team providing General Medical Services to the patients of the Practice, including treatment room care and chronic disease management.

Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs

To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice

To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients’ health

To request pathology services and process pathology results as required

Chronic disease management: Diabetes, COPD, Asthma and Contraception

To ensure compliance with the practice chaperone policy

To assist GPs with minor surgery when required

To prioritise health issues and intervene appropriately

To support the team in dealing with clinical emergencies

To provide a range of nursing services, including cytology, wound care, doppler, travel services, CHD/stroke monitoring, baby clinics, vaccinations and immunisations, health checks, suture removal, assist GPs with minor surgeries and coil fittings etc.

To ensure read codes are used effectively

Comply with QOF, enhanced services and medicine management initiatives to maintain high standards

Other tasks will include:

Maintaining and cleaning equipment used by the nurses and GPs

Support the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to extant legislation

Chaperoning and assisting patients who are being examined by another clinician

Maintaining general tidiness and cleanliness of nurses and treatment rooms

To support the clinical team with all safeguarding matters, in accordance with local and national policies

At all times there will be a need to maintain accurate records

A duty to advise senior nurse of potential problems or errors within the range of assigned tasks

Attend and participate in any Practice meetings when required

Attendance at ICB and practice led clinical education meetings and training

Any other delegated duties appropriate to the post
