NURSE PRACTITIONER – permanent (due to expansion)

Due to expansion, we are looking for another Nurse Practitioner to join our team working 3 days/week

Bramblehaies Surgery is a well organised and cohesive practice with a friendly working environment, quality care and a proven record of innovation and adapting to change. Situated in Cullompton, we own purpose-built premises with an on-site primary health care team, including our own Health Facilitator and close links with our community team with some providing clinics in the surgery. We have a staff room for informal chats over hot drinks and lunch, and a meeting room for our regular clinical and practice meetings.

The practice has an expanding list of 7100 patients served by 3.125 whole-time equivalent GPs; four Partners and one Salaried GP. At least one GP does a duty surgery each day, working alongside our Nurse Practitioner, plus the usual supporting staff.

The successful applicant will join our expanding team, working autonomously but with the opportunity to influence the future direction of the Practice.

The Practice currently maximises capacity via our duty system daily and our work on self-care and social prescribing. The reception team actively navigate patients to see the right professional at the right time. As a result, less than 40% of our telephone consultations convert to face-to-face attendances. This gives us control over our ‘urgent’ workload and means that patients are dealt with on the day. Work is ongoing with routine appointments – we don’t have the magic answer on that at present!

Clinical administration is kept manageable by the involvement of the whole Practice team. The reception team contact patients with path results and the admin team scan and code hospital correspondence, work flowing relevant correspondence to the GP, all adhering to protocol. Medication is issued via EPS with the practice making a concerted effort to actively promote eRD. Our Nurse Practitioner assists with hospital discharge and meds reconciliation, along with INR star management. 

Home visiting is shared amongst the GPs and Nurse Practitioner, with a weekly visit by a GP to the brain injury rehabilitation unit.

We’re busy – but not drowning – and the workload is manageable.

We have excellent nurse-led chronic disease management and achieve high QOF points, CQC rate us Good. We are a SystmOne practice, and continually embrace new ways of working to ensure we utilise the system saving resources. We are currently undergoing assessment to become a training practice.

The post would suit an established NP seeking to join an established team, although opportunities for development are available for relatively newly qualified practitioners.
