Lipid Pathway Fellowships – Somerset Training Hub

An exciting opportunity has arisen for the recruitment of two Lipid fellows working 1-2 days week in Somerset.
We are ideally looking for primary care nurses to support the current lipid clinic and improve identification of patients with, or at high risk of, familial hypercholesterolaemia (FH). will also consider other allied healthcare professionals working within primary care.
FH is a genetic disease which results in high blood cholesterol and an increased risk of early cardiovascular disease (CVD).
The Lipid Pathway Fellow is a 1-year training post to:
- Support primary care towards detection and management of hyperlipidaemia, with a countywide focus
- Work with the training hub to develop educational tools to help support primary care in managing lipidaemia
- Work with the county lead clinician to develop novel ways of delivering the current service
- Work with the lipid and FH services to develop pathways of care from primary to specialist services
The Lipid Pathway Fellow will be supported by:
Somerset Training Hub
Secondary care lipid management team
South West Academic Health Science Network (AHSN)
Public Health Somerset
South West Genomic Laboratory
Duties and responsibilities would include.
- Supporting communication and dissemination of information related to the lipid management
- With support, undertake evaluation data collection to demonstrate impact of fellowship post
- Engage with key ICS and regional clinical commissioning leads to share learning of barriers and opportunities, including the Southwest AHSN, Genomics Medicine Service Alliance and Office of Health improvement
- Engage with the current lipid team to develop pathways
- Engage with the training hub to develop learning tools for primary care
- Undertake the Online primary care FH course in Northumbria University
- Taking responsibility for ongoing personal and professional development
- Spend time with your designated supervisor who will mentor you through this work
