Individual Funding Requests Panel (IFR) Application Form

The CCG in consultation with the LMC have made amendments to the IFR application form. You should find the new form self-explanatory, and that it guides you to the important aspect of exceptionality for your patient. You will notice that a number of questions have been removed. The form can either be accessed on the GP Pathway navigator orto access the IFR application form on the Somerset CCG website Individual Funding page you will need to sign in as a health professional (access by GP practices is by using their practice username and password details); https://nhssomerset.nhs.uk/evidence-based-interventions/
As you may appreciate, although each GP or specialist may only make few applications, the Panel sees all of them. It is really helpful to the Panel to receive all the clinical correspondence (both GP referral and all specialist letters and results) related to your application in the first place, as well as photos if requested in the Policy Guidance.Please note this form is not to be used when applying for Fertility treatment funding.
