It is advised that 2 or 3 practices within the same locality cluster together to attend this training, please could the lead practice manager from the cluster make the booking.

Please note the training session start time can be tailored to suit your cluster but should be between the times stated above, we will then confirm that this time has been allocated to you. Sessions will be up to 2 hours in duration.

Objectives for the session will be agreed with each lead PM and are expected to include

1.       Summary of key principles and approaches of successful active signposting

2.       Sharing of progress, successes and problems since initial training

3.       Identifying helpful information resources, support and skills

4.       Working through typical patient requests and problems to agree actions and practice difficult conversations

Any Somerset practice staff carrying out active signposting will benefit from attending but service commitments will mean that individual practices will decide who is able to attend the session.  Management and executive support is an essential part of successful active signposting and either relevant involvement on the day or follow-up communication with Dr Martin Hughes will be helpful.
