NHS England Performance Review Process: After a presentation from the NHS England Professional Performance team at a recent meeting of South West Regional LMCS it became apparent that many GP colleagues were not aware of this process, we have therefore put together a webpage on the performance process and the LMC involvement which we hope you will find informative and reassuring.

Oliver McGowan Mandatory Training: Practices will be aware that following the enactment of the Health and Care Act 2022 on 1st July 2022, it became a requirement for CQC-registered service providers to ensure that all employees receive the Oliver McGowan Mandatory Training in Learning Disability and Autism, appropriate to their role. HEE have informed us that a Code of Practice is now being drawn up and once that is in place a full procurement can be undertaken for the long-term delivery of the training. The intention is to make the Tier 1 training available over the summer and for Tier 2 to be available once the full infrastructure is in place. Full communications will be released once they are certain of the model to be used and how this will be managed. There is no requirement at this stage for any service provider to make its own arrangements.

Laboratory Tests from Merged Practices (a plea from the Lab): When requesting laboratory tests using a manual (paper) request form, please do ensure the GP location given is the current (merged) practice name. Using the previous individual surgery name causes confusion when the requests are booked in and could cause delays in receiving the test results. Where possible, please avoid using manual requests for laboratory tests altogether. If you are unable/unsure of how to request a certain test in tQuest, please contact the laboratory or duty.biochemist@somersetft.nhs.uk for advice.

CQC Consultation: The Government are seeking feedback from all providers of a regulated activity that are registered with the Care Quality Commission (CQC) in England to determine:

Hospital@Home: You will all be aware by now that the Government has started a national scheme to set up virtual wards in all English Integrated Care Systems to increase capacity before we hit next winter, and also, we presume, to see if care can be provided better, faster, and with fewer staff. Somerset ICS has been allocated a little over £2M for first year set-up costs and matched funding for the second year. Thereafter we assume H@H will have to funded from within existing budgets.
The Somerset scheme will ultimately allow some 240 patients to be “admitted” with either respiratory illness or conditions linked to frailty. The choice of the first group is eminently logical given the experience of home monitoring of Covid patients, and years of both outreach and practice based care for patients with Asthma and COPD. Frailty is going to be more complicated, but it will also offer great potential benefits in keeping older people out of hospital and in their own homes.
The timescale for setting all this up is challenging and a lot of things need to be done simultaneously including developing referral criteria and pathways, care package requirements, treatment options, staff skill mix, and prescribing and discharge protocols, amongst others. A very small trial frailty pilot involving one assessing clinician is starting in Bridgwater immediately, and as the scheme winds up more areas will progressively be added. Effective liaison with existing complex care and extensivist service will, of course, be essential.
The LMC thinks that H@H could offer benefits both for patients and the broader health and social care system, and we are encouraged by both its clinical leadership and the willingness of the ICB team leading it to listen to the LMC concerns and to engage with practices in the design process.

Linking up our General Practice Paramedics: If practices have paramedics joining their teams our LMC/Training Hub paramedic ambassador Catherine Hayden is very keen for them to get in touch. Most paramedics will already be linked in to the email updates and support forums, but new faces are popping up all the time who may be unaware of the support and training opportunities we can provide. As we move toward HEE First Contact Practitioner and Advanced Practice status, Catherine is also happy to offer advice to paramedics and employers of options available for both portfolio or taught routes. If you have any paramedics in your team who would like to be included or have any questions, please contact catherine.hayden@nhs.net

Young Somerset: Are happy to talk with GPs to establish closer links with the Mental Health and Wellbeing Service to ensure coverage across the county.

As well as recruiting for GP host practices there are a number of other opportunities within the medical school. We are currently recruiting Academic Tutors (who meet with students over the course of a year to mentor and help support student learning) as well as a Rheumatology clinical teaching lead that might suit someone with a special interest. I'm always happy to chat things through and work flexibly with practices or individuals so please don't hesitate to get in touch. "
