Post-COVID-19 Syndrome (also known as Long COVID) has around 200 connected symptoms. Breathlessness is one of the most common. Most people with Post COVID have normal chest examinations and investigations, but ongoing breathlessness. The breathlessness often has a predictable onset e.g. with certain activities. In these circumstances the cause of this breathlessness is usually “breathing pattern disorder” (BPD). This used to be called “dysfunctional breathing”. BPD is connected to dysautonomia (autonomic dysfunction), including tachycardia. A diagnosis of BPD can be aided by completing the nijmegen questionnaire. BPD does not respond well to medication but is treated with breathing retraining.
Retraining focuses on “nose, low, slow” breathing. In BPD, inhalation is via the mouth, there is increased use of respiratory/chest muscles and breathing rate increases. Retraining aims to focus on:
1. “Nose” breathing - inhalation of air through the nose.
2. “Low” breathing – encouragement of abdominal breathing by advising the patient to place their hand on their abdomen and feel the abdominal expansion as they inhale. This increases diaphragmatic, rather than chest muscle, breathing.
3. “Slow” breathing – consciously slowing breathing to a comfortable rate to increase efficiency.
We recommend that people with BPD become familiar with this technique when they are not short of breath. They can do this by practicing “nose, low, slow” breathing for at least 5 minutes in the morning and evening. The technique then becomes easier to implement should the breathing pattern become disordered.
The British Lung Foundation have excellent patient videos to help retraining. You may want to look up the videos yourself (they are a few minutes long) and also direct patients to them.

Message from David Long - SPIROMETRY - I do not know the finer detail as yet but will send then as soon as I know but a service is planned to start in March of next year which will involve FeNo and Spirometry as part of the investigation that can be requested. Funding is available for you to undertake a spirometry course and if you are interested, please contact rachael.rowe@nhs.net PULMONARY REHABILITATION AND QOF - Just a reminder to meet QOF target you only have to offer PR and tick the box; you don't have to refer, so if the patient declines you still meet the target.
