GP Board Transformation Funding A number of practices have asked whether Transformation Fund money is available for local projects. Because of time pressure and the need to ensure equity the 2018-19 allocation has already been agreed between the CCG and the GP Board for the following purposes

• Create Vibrant New Working Arrangements

• The Ten High Impact Actions

• Service Improvement Function ( LMC practice support visits)

• Supporting GPs to Work In Multidisciplinary Teams

• Creating a strong provider voice ( Somerset GP Board)

• Development of SPH

The full value of funding received to date is committed to these projects , but as Primary Care Networks evolve the GP Board anticipates that there are likely to be additional funding streams in future, though we do not know who will be responsible for spending this money. If your practice or PCN would like to propose a scheme in case money does arrive for short term allocation, please do make the GP Board aware so we help in such circumstances . GP Practices directing patients to laboratories for cervical screening results: The Screening and Immunisation Team tells us of incidents where practices have given patients the cytology laboratory’s telephone number to find out their cervical screening and colposcopy biopsy results and discuss any follow up treatment. Laboratory staff are not able to discuss any pathology reports or recall information with patients for information governance reasons and because they are not the patient’s clinician. Surgery staff can however contact the lab. on patients’ behalf if needs be. We have been asked to make sure that all your staff know about this.

NEW MATERNITY SELF-BOOKING AT T&S On Monday 11th March the Maternity Digital Transformation programme launched a new online self-referral form on the T&S website maternity for newly pregnant women to register for maternity services. This will allow their admin. team to complete pre-registration information required for clinical systems as they prepare to offer a ‘paper-light’ clinical documentation system, Medway Maternity. The form collects basic demographic details alongside basic obstetric and medical history. Once a self-referral has been submitted a midwife will contact the patient directly to offer early pregnancy advice and to arrange an appointment at a suitable time and place. When a pregnant lady presents at your surgery to request a booking appointment please also direct her to complete the online form services to allow direct communication with her midwife before her appointment.
