Health authorities should prepare local action plans to reduce violence in general practice. Health authorities, trusts and general practices can adapt the procedures for dealing with racially abusive patients outlined below. If there are incidents of racial abuse, doctors will know how they can respond. A clear policy statement means that those who think they are being racially harassed and those who witness others being racially harassed, will be able to raise the matter through the organisation's procedures. Doctors should feel confident that they work in a supportive environment. They should feel able to speak out against unacceptable behaviour and support their colleagues. Doctors must also behave in an appropriate manner towards their colleagues and patients. It is important that the organisation is seen to have acted, as far as possible, in such a way as to guarantee that the doctors and other staff for whom it is responsible are not subjected to degrading treatment or to other infringements of their Convention rights in the course of their work. The policy statement must be directed at patients, as well as service users, carers, relatives and members of the public. It should be displayed in prominent positions throughout the premises, on patient information leaflets, and on hospital and practice appointment cards. The statement should include the possible action that the health authority/trust/general practice may take against violent and racially abusive patients. It should state clearly that abuse and violence of any kind will not be tolerated. The approach to patients who commit acts of violence (as defined above) or who harass doctors might be either informal or formal.[12] [13] Doctors who are the subject of racial harassment or other violence should first consider an informal approach. If they believe the incident is severe, they could immediately use the formal action. They should follow the reporting mechanism which should be in place for recording and monitoring such incidents. Psychiatric patients may be violent or abusive to health care workers. This may be a symptom of their mental illness. Doctors should still be able to use the informal action outlined below. Other local procedures have been introduced for dealing with incidents involving patients with mental disorders.[14] Doctors need to take care to ensure that the refusal of treatment is done in a way that does not amount to an infringement of the patient's other Convention rights, e.g. Articles 2 (right to life) and 3 (right not to be subjected to degrading treatment or punishment).
